Description

get_terminal_tools() is annotated (and used in middleware.py) as returning tuple[dict[str, dict], Optional[str]]

Several paths (mostly the failure paths) do not actually return a tuple and middleware.py throws exceptions because of it.

Fixed

Fixes exceptions thrown in middleware.py due to invalid return values.
