Changelog Entry

Description

The chat_message write in upsert_message_to_chat_by_id_and_message_id fails silently (only log.warning). When it does, the new message lands in the chat JSON blob but not in the chat_message table. On the next turn, get_message_list finds nothing in the incomplete rows and the LLM only receives [system].

load_messages_from_db falls back to the embedded chat history when the chain is incomplete, then backfills the missing rows so subsequent requests use the fast path. Mirrors the existing gap-healing in get_messages_map_by_chat_id.
