  1. Quizzes need correct numbering. For some reason, it doesn't pick up the chapter number. I wonder if I am generating the DIV correctly or not.
  2. Also, the "See Answer" isn't aligned nicely under the number. It either starts to the left or under the bullet; I can't seem to get it to align correctly. The link itself is OK.
  3. While the quiz shows for HTML, it doesn't appear for PDF.

All three issues need to be resolved.
