Prisma on MongoDB matches a `null` filter only against stored nulls. Better Auth leaves optional fields out on create, so `eq null` lookups missed those records, including guarded `update`/`incrementOne` calls used by refresh-token rotation, device-code claiming, SCIM, and API keys. Also make the memory adapter's `ne null` skip unset fields, and simplify oauth-provider `findConsent` now that `eq null` is reliable.
