0202b289c2 assorted small netmgr-related changes 
- rename isc_nmsocket_t->tcphandle to statichandle
- cancelread functions now take handles instead of sockets
- add a 'client' flag in socket objects, currently unused, to
  indicate whether it is to be used as a client or server socket

761d135ed6 [master] add TCP pipelining support 
4040.	[func]		Added server-side support for pipelined TCP
			queries. TCP connections are no longer closed after
			the first query received from a client. (The new
			"keep-response-order" option allows clients to be
			specified for which the old behavior will still be
